Application of Rapidplan Optimization Module in Cervical Cancer Volume Modulation Arc Plan

Abstract:Objective To explore the feasibility and potential advantages of Varian Rapidplan optimization module in volumetric
modulated arc therapy (VMAT) plan of cervical cancer. Methods Fifty cases with clinically verified cervical cancer VMAT plan were
selected and imported into the Rapidplan database to establish an automatic optimization model. Then, another 12 cases with cervical
cancer VMAT plan were used to test the model. The dosimetric differences of target area and organs at risk (OAR) were compared
between Rapid-plan, an automatic plan using Rapidplan optimization module, and conventional Clinical-plan. Results Rapid-plan
could predict the dose distribution well. Compared with Clinical-plan, Rapid-plan significantly improved the homogeneity index (HI)
(1.0597±0.01058 vs. 1.0459±0.0079) and conformity index (CI) (0.7927±0.6686 vs. 0.8275±0.03788) of target areas. Rapid-plan
had more advantages in the protection of OAR. Rapid-plan significantly reduced Dmax of the spinal cord, V20, V30, and V40 of the
small intestine, rectum, sigmoid colon, and femoral head. In addition, in terms of optimization efficiency, the average optimization
time of Rapid-plan was 11.45% shorter than that of Clinical-plan. In terms of execution efficiency, the average MU in Rapid-plan was
reduced by 62 MU, which was 10.58% lower than that in Clinical-plan. Conclusion Rapidplan optimization module can efficiently
complete the VMAT plan of cervical cancer. It is recommended to prepare radiotherapy plan for cervical cancer planning when using
Varian Eclipse planning system.
