Research on Transcranial Ultrasound Perfusion Imaging in Patients
with Severe Internal Carotid Artery Stenosis or Occlusion

Abstract:Objective To explore the characteristics and significance of transcranial ultrasound perfusion imaging (UPI) in
assessing the cerebral perfusion in patients with internal carotid artery (ICA) severe stenosis or occlusion. Methods A total of 55
patients with severe unilateral ICA stenosis or occlusion were erolled in this study. All the patients received UPI and computed
tomography perfusion (CTP). The time to peak enhancement (TPE), microcirculation blood flow velocity (β), microcirculation
blood volume (A) and local cerebral blood flow (A×β) of bilateral basal ganglia ROI were compared between bilateral basal
ganglia. ROC was employed to determine the threshold value and calculate the accuracy, sensitivity, positive predictive value and
negative predictive value. Results The TPE in the basal ganglia of the affected side and the healthy side were (23.16±4.09) s and
(21.07±3.49) s, respectively, and the difference was statistically significant (P<0.05). β-values in the affected side (1.19±0.87) s-1
was slower than that in the healthy side (1.66±1.18) s-1 (P<0.05). A×β in the affected side (3.21±3.68) dB/s was lower than that in
the healthy side (4.83±5.27) dB/s (P<0.05). The AUC of β, A×β and TPE was significantly higher than 0.5 (P<0.05). When β<1.07,
the sensitivity was 70.9%, and the accuracy was 66.4%. When TPE>22.10 s, the specificity was 67.3%. When A×β<1.07 dB/s, the
sensitivity was 85.5%, and the positive predictive value was 74.2%. Conclusion UPI can quantitatively evaluated the perfusion
of patients’ brain tissue with unilateral severe ICA stenosis or occlusion. When the TPE is longer than that of the contralateral, the
velocity of microcirculation blood flow β is slower than that of the contralateral, and the local tissue blood flow A×β is lower than
that of the contralateral, we can consider the existence of intracranial hypoperfusion.
