摘要

目的 评估双向Glenn手术(Bidirectional Glenn Shunt,BGS)术后肺动脉血流动力学改变以及血管切应力特性,分析 不同振荡切应力数据对评价Glenn术后恢复情况的应用价值。方法 通过磁共振扫描成像,采集8例BGS术后的肺动脉影像, 并采用计算流体动力学法获得重建三维BGS模型切应力和振荡切应力。结果 本次研究的8例先心患儿计算获得平均振荡切 应力为(0.233±0.067)Pa。病例7中,上腔静脉和左肺动脉存在低切应力的情况,计算振荡切应力最低值为0.14 Pa。结论 BGS术后利用计算流体动力学评价患者肺动脉个体化切应力模型和平均振荡切应力有助于了解患者血流动力学情况。

Abstract

Objective To evaluate the hemodynamic changes and the shear stress characteristics of the pulmonary arteries after bidirectional Glenn shunt (BGS) and assess the value of different oscillatory shear stress after the surgery. Methods The pulmonary artery images of 8 congenital heart disease patients after BGS were collected using magnetic resonance imaging. The threedimensional shear stress model and the oscillatory shear stress value were calculated by computational fluid dynamics. Results In this study, the average oscillatory shear stress was (0.233±0.067) Pa in these 8 congenital heart disease patients. In case 7, there was a low shear stress in the superior vena cava and the left pulmonary artery, and the lowest value of the calculated oscillatory shear stress was 0.14 Pa. Conclusion For congenital heart disease patients after BGS, evaluating personalized shear stress model and the mean oscillatory shear stress of pulmonary artery based on computational fluid dynamics contributes to the understanding of the hemodynamics of the patients.
