Abstract:Objective To explore the application value of metal artifact reduction (MAR) technique in gemstone spectral imaging
follow-up after spring coil embolization intervention in patients with portal hypertension. Methods The gemstone spectral imaging
data of 42 patients who had inserted a spring coil in the upper abdomen after interventional surgery were collected retrospectively
and reconstructed in group A and group B. Group A was images without MAR technique, and group B was images with MAR
technique. In group A, region of interest (ROI) was delineated in the abdominal aorta with the deepest color of the artifact, the liver
and spleen affected by the artifact, and the left psoas muscle that was not affected by the artifact, and copied into the images of group
B. The signal to noise ratio (SNR), contrast to noise ratio (CNR), artifact index (AI) and beam hardening artifact removal rate (BAR)
were calculated respectively; Wilcoxon rank test of paired samples was performed on SDmean (average of abdominal aorta, liver,
and spleen), SD4 (psoas muscle), CNR and AI of the two groups of images. Two senior diagnosticians scored the images of the two
groups on a 4-point system; after the consistency test of scores, the different scores were discussed to determine the final scores, and
the final scores of the two sets of images were was performed Wilcoxon rank sum test. Results With MAR technique, The SDmean and
AI values decreased significantly after the application of MAR technology [16.640 (8.374) for group A vs. 12.016 (3.411) for group
B, 14.599 (8.189) for group A vs. 7.439 (5.325) for group B, all P<0.01]; there were no significant difference in SNR, CNR and SD4
[11.755 (3.319) for group A vs. 12.142 (2.905) for group B, 5.893 (3.226) for group A vs. 6.085 (2.338) for group B, 9.504 (2.191)
for group A vs. 9.657 (2.432) for group B, all P>0.05]; the BAR was 48.9% (24.4%); the subjective scores of the two physicians
had good consistency (the Kappa values were 0.820 and 0.840 respectively), and MAR technique could significantly improve the
subjective score of the image (P<0.01). Conclusion During the CT follow-up of the patients who underwent interventional surgery
to implant coil in the upper abdomen, the MAR technique of gemstone spectral imaging CT can significantly reduce metal artifacts
and improve the image quality.
