Study on the Correlation Between 18F-FDG PET/CT Metabolic Parameters and
Immune Cells in Non-Small Cell Lung Cancer

Abstract:Objective To analyze the correlation between metabolic parameters of 18F-fluorodeoxyglucose (18F-FDG) positron
emission tomography/computed tomography (PET/CT) imaging and infiltrating immune cells and peripheral blood immune cells
in patients with non-small cell lung cancer (NSCLC). Methods The clinical data of 83 patients who received 18F-FDG PET/CT
examination in our hospital from March 2016 to June 2020 and were newly diagnosed as NSCLC were retrospectively analyzed.
The expression of infiltrating immune cells in the primary tumor was detected by immunohistochemistry and preoperative peripheral
blood immune cell levels were detected. Spearman rank correlation was used to test the correlation between PET/CT metabolic
parameters and the expression levels of infiltrating immune cells and peripheral blood immune cells, respectively. Results Maximum
standardized uptake value (SUVmax) and mean standardized uptake value (SUVmean) were significantly positively correlated with
CD163+TAMS and CD8+TILs expression levels (P<0.05), but not significantly correlated with CD68+TAMS, CD11c+DCS, CD3+TILs
and Foxp3+Tregs expression levels (P>0.05). The expression levels of metabolic tumor volume (MTV), total lesion glycolysis
(TLG) were significantly positively correlated with Foxp3+Tregs in primary tumors (P<0.05), but not significantly correlated with
the expression levels of CD163+TAMS, CD8+TILs, CD68+TAMS, CD11c+DCS and CD3+TILs (P>0.05). SUVmax, SUVmean and MTV
were significantly positively correlated with neutrophil to lymphocyte ratio (NLR) (P<0.05), but not significantly correlated with
platelet to lymphocyte ratio (PLR), CD3+, CD4+/CD8+ and natural killer cell (NK) (P>0.05). There was no significant correlation
between TLG and NLR, PLR, CD3+, CD4+/CD8+ and NK (P>0.05). Conclusion The metabolic parameters of 18F-FDG PET/CT
in NSCLC are significantly correlated with some infiltrating immune cells and peripheral blood immune cells, suggesting that the metabolic parameters of 18F-FDG PET/CT can reflect the local immune status of tumor to some extent, and have important reference
value for the clinical application of immune checkpoint blockers in NSCLC patients.
