Abstract：Objective To explore the changes of medical expenses, medical quality and efficiency in department of ophthalmology
of a grade A hospital in the past one year since the implementation of the purchase policy of intraocular lens with
volume. Methods A database was established based on the front page of the hospital’s medical records, medical expenses,
and surgical indicators, and patients who underwent ophthalmic lens implantation were selected as the research objects. From
January to November 2020, before the implementation of artificial lens procurement (n=927), and from January to November
2021, after the implementation (n=925), STATA 14.0 was used to perform rank sum test on the medical expenses, cost structure,
average hospitalization days, preoperative waiting days, and operation time of ophthalmic artificial lens surgery before and after
the implementation. The χ2 test of postoperative infection and satisfaction of patients was used to explore the differences in various
indicators of ophthalmic lens surgery before and after the implementation. Results After the implementation of volume procurement,
the hospitalization expenses and consumables expenses of patients undergoing ophthalmic intraocular lens surgery decreased (P<0.05).
The structure of other expenses (medicine, examination, surgery and other expenses) has not changed significantly (P>0.05). In
addition, before and after the implementation of intraocular lens surgery in patients with the average length of stay, preoperative
waiting days, operation time, postoperative infection and satisfaction were not statistically significant (P>0.05). Conclusion After
the implementation of ophthalmic intraocular lens, the burden of medical expenses of patients decreased, and the pressure of medical
insurance fund was alleviated to some extent. The implementation of volume procurement does not affect the medical efficiency and
quality of ophthalmic intraocular lens surgery. It is a medical insurance health policy worthy of promotion. It is hoped that certain
incentives will be implemented for ophthalmic intraocular lens surgery to better complete the volume procurement goal.
