Meta-Analysis of the Clinical Application of Irreversible Electroporation Ablation for Liver Cancer

Abstract:Objective In 2011, the FDA approved a NanoKnife for tumor ablation using irreversible electroporation. In order to
evaluate the feasibility of this technique, the safety and efficacy of clinical application were analyzed. Methods Reports about
clinical treatment of liver cancer with NanoKnife were sought in the PubMed, Web of Science, Cochrane Library, Embase, Scopus
databases from their inception until June 2019. Three independent researchers screened the selected reports and conducted feature
extraction. Version 14.0 of the Stata software was used for the meta-analysis. Results Eighteen reports were identified covering 565
participants and 771 lesions. The meta-analysis showed that irreversible electroporation technique reduced the complication rate
[95%CI: 26% (18%, 38%)] and recurrence rate [95%CI: 36% (28%, 46%)] after tumor treatment, and improved complete response
rate [95%CI: 840% (503%, 1403%)]. Conclusion Irreversible electroporation technique has advantages in tumor ablation adjacent to
blood vessels and nerves, but research on electrical pulse optimization and preoperative simulation needs to be strengthened.
