Application of Spatio–Temporal Image Correlation Technique and Tissue Doppler Imaging Technique in Evaluation of Right Ventricular Function of the Fetus with Nuchal Cord
Abstract:Objective To investigate the application value of spatiotemporal image correlation (STIC) combined with tissue Doppler imaging (TDI) in evaluating the right ventricular function of fetus with umbilical cord around neck. Methods A total of 98 cases of umbilical cord around the neck in our hospital were selected as the research object. According to the number of umbilical cord around the neck, they were divided into A group (umbilical around the neck for one week) and B group (umbilical around the neck for two weeks or more). In addition, 143 cases of fetus without umbilical cord around the neck in the same gestational week were selected as the control group. The right ventricular end systolic volume (RVESV) and right ventricular end stroke volume (RVEDV) were obtained by STIC technique. Then the right ventricular stroke volume (RVSV), right ventricular cardiac output (RVCO) and right ventricular ejection fraction (RVEF) were calculated. Doppler spectra of tricuspid annulus tissue were obtained by TDI technique. EA peak in early diastolic phase, Aa peak in late diastolic phase and Sa peak in systolic phase were recorded, and Ea/Aa value and Tei index were calculated. The results of the above indicators were compared and analyzed among the three groups. Results Compared with the
control group, there was no significant difference in RVESV, RVEDV, RVSV, RVCO and RVEF in A group (all P>0.05), while the
values of Ea, Aa and Ea/Aa were decreased (P<0.05), and Tei index was increased (P<0.05). Compared with the control group and A group, the RVESV, RVEDV, Tei index of B group were increased, while RVSV, RVCO and RVEF were decreased, the differences were statistically significant (all P<0.05). In addition, Ea, Aa and Ea/Aa in B group were significantly lower than those in control group (P<0. 05), but there was no significant difference between A group and B group (P>0.05). Conclusion STIC combined with TDI can accu- rately evaluate right ventricular systolic, diastolic and global function of the fetuses with nuchal cord, which can provide important information in guiding the active clinical treatment.
