Diagnostic Value of Vascular Plaque Quantification Based on Three Dimensional Ultrasound
in Carotid Vulnerable Plaque

Abstract:Objective To explore the value and feasibility of 3D ultrasound vascular plaque quantification (3D US VPQ) to evaluate the
nature of carotid plaque. Methods A total of 47 carotid plaque from carotid endarterectomy were analyzed retrospectively. Quantitative
analysis of vascular plaque was performed using 3D US before surgery, and then compared with postoperative pathology. The
differences in various parameters before and after the operation were compared, and the receiver operating characteristic (ROC)
curve of median gray and standardized wall index was constructed to evaluate the diagnostic value of 3D US. Results LDL
in vulnerable plaque group was higher than that in stable plaque group (t=1.581, P<0.001). The gray scale median (GSM) of
vulnerable plaque was significantly lower than that of stable plaque group (t=5.076, P<0.001). The normalized wall index (NWI)
of vulnerable plaque was higher than that of stable plaque (t=2.314, P=0.025). There was no significant difference between the
two groups in plaque area stenosis rate, plaque thickness and plaque volume (P>0.05). The sensitivity, specificity, accuracy,
positive predictive value and negative predictive value of GSM were 68.0%, 81.8%, 74.5%, 81.0%, 69.2%. Those of NWI were
60.0%、81.8%、70.2%、78.9%、64.3%. The areas under the ROC curve of GSM and NWI were 0.749 and 0.709. Conclusion
Using GSM and NWI, 3D US VPQ can quantitatively assess plaque composition and vascular load, which is of great value in the
diagnosis of vulnerable plaques.
