Comparison of MRI and CBCT in Diagnosis of Periapical Lesions

Abstract:Objective To explore the application value of magnetic resonance imaging (MRI) and cone beam computed tomography
(CBCT) in evaluating periapical lesions. Methods A total of 20 patients with apical periodontitis (AP) who underwent MRI and
CBCT in our hospital from February 2022 to February 2023 were selected. The lesions were compared between T1WI, T2WI images
of MRI and CBCT, including lesions of width, height, and area. Results The 35 lesions displayed on CBCT in 20 patients with
chronic AP were clearly visible on MRI images. CBCT could display mineralized tissue components, such as teeth and bones. MRI
could provide information on soft tissue components. MRI images provided diagnostic details to evaluate the main structure of
tooth hard tissue. Uneven contrast was observed within the lesion, usually with significant enhancement near the apical foramen and
periodontal spaces. There was no difference in lesion visibility was observed between MRI and CBCT, and the lesion size was well
matched. There was a high correlation between the lesion size displayed on T1WI and T2WI images of MRI examination (P<0.01).
The lesion size displayed on T1WI and T2WI were no significant differences (P>0.05). The lesion size displayed on T1WI and T2WI
images were larger than CBCT, the difference was statistically significant (P<0.05). Conclusion MRI can provide soft tissue features,
which can help detailed analysis of periapical lesions. CBCT combined with MRI can better understand the extent of periapical
lesions and pathological changes, thereby confirming the applicability of comparative MRI in the identification of periapical lesions.
