Abstract:Objective To investigate the predictive value of magnetic resonance imaging (MRI) combined with cerebrospinal fluid
soluble interleukin-2 receptor (SIL-2R) and calcitonin gene-related peptide (CGRP) levels for poor prognosis in children with severe
viral encephalitis. Methods A total of 129 children with severe viral encephalitis treated in our hospital were selected, and they were
divided into good prognosis group (n=88) and poor prognosis group (n=41) based on their prognosis. The abnormal rate of MRI and
the levels of SIL-2R and CGRP in cerebrospinal fluid at admission were compared between the two groups, and Logistic regression
analysis method was used to analyze the influencing factors of poor prognosis in children with severe viral encephalitis, and the
receiver operating characteristic curve was used to analyze the predictive value of MRI abnormalities and the levels of SIL-
2R and CGRP in cerebrospinal fluid for poor prognosis. Results The abnormal rate of MRI and the levels of SIL-2R and CGRP in
cerebrospinal fluid in the poor prognosis group were higher than those in the good prognosis group (P<0.05), and the duration of
convulsions and the course of disease were longer than those in the good prognosis group (P<0.05). Multivariate Logistic regression
analysis showed that duration of convulsions, the course of disease, the abnormal rate of MRI and the high expressions of SIL-2R
and CGRP in cerebrospinal fluid were all risk factors for poor prognosis in children with severe viral encephalitis (P<0.05). The
sensitivity and area under the curve of MRI combined with the levels of SIL-2R and CGRP in cerebrospinal fluid in predicting the
poor prognosis of children with severe viral encephalitis were higher than those of each single detection (P<0.05), but there was no
significant difference in the specificity between the combination and each single prediction (P>0.05). Conclusion MRI abnormalities
and the levels of SIL-2R and CGRP in cerebrospinal fluid have certain prediction efficacy for the poor prognosis of children with
severe viral encephalitis, and the combined prediction efficiency is higher, which can be used to guide clinical treatment.
