Abstract:Objective To investigate the value of diffusion kurtosis imaging (DKI) in combined with prostate-specific antigen
(PSA) in evaluating prostate cancer (PCa) invasiveness. Methods A total of 55 patients with PCa, all with PSA findings and DKI
magnetic resonance imaging were analysed for the spearman correlation between DKI parameters and the Gleason score (GS).
The differences of DKI parameters between clinically significant prostate cancer (csPCa) and non-clinically significant prostate
cancer groups were compared by independent sample t-test and Wilcoxon rank sum test. Finally, a diagnostic model was established in
combination with PSA to investigate the diagnostic value of single and combined models for PCa aggressiveness. Results Mean diffusion
(MD), axial diffusion (Da), radial diffusion (Dr) values were negatively correlated with GS, while fractional anisotropy (FA), mean kurtosis
(MK), axial kurtosis (Ka), radial kurtosis (Kr) values were positively correlated with GS (all P<0.05). The differences in FA, MD, Da,
Dr, MK, Ka and Kr between different GS groups were significant; the combined model (DKI+ PSA) had the highest diagnostic efficacy,
with an area under the ROC curve of 0.883. Conclusion All DKI parameters have significant advantages in distinguishing csPCa from
non-csPCa, and can be used to determine the invasiveness of PCa; combined model (DKI+ PSA) has the best diagnostic capability.
