Abstract:Objective To explore the predictive value of an early-warning model for ischemic stroke risk in patients with carotid
atherosclerosis which constructed based on high-resolution MRI. Methods A total of 80 patients with carotid atherosclerosis who
were screened by B-ultrasound in our hospital from January 2019 to March 2022 were collected. The incidence of stroke was
recorded after 60 days of follow-up, and the patients were divided into non-stroke group and ischemic stroke group. Clinical data
such as age, gender, systolic blood pressure, diastolic blood pressure, smoking or not, complicated with hyperlipidemia, diabetes,
hyperhomocysteinemia, and MRI plaque load parameters were compared between the two groups. Meanwhile the differences of
laboratory indexes and plaque load indexes between the two groups were compared. Multivariate Logistic regression model was used
to analyze the risk factors of ischemic stroke in patients with carotid atherosclerosis, and a early-warning model was established.
The receiver operating characteristic (ROC) curve was used to evaluate the effectiveness of the risk early-warning model in ischemic
stroke. Results Stroke events occurred in 15 of the 80 patients, with an incidence rate of 18.74%. There were significant differences
in the hyperlipidemia, diabetes, low-density lipoprotein cholesterol, total cholesterol, intra-plaque hemorrhage, lipid rich necrotic core
(LRNC), fibrous cap rupture (FCR), and remodeling index (RI), wall thickness, normalized wall index (NWI) of common carotid
artery, and RI, wall thickness of internal carotid artery between stroke group (n=15) and the non-stroke group (n=65) (P<0.05). The
results of multivariate Logistic regression analysis showed that LRNC, FCR and the NWI and RI of common carotid artery were the
risk factors for ischemic stroke in patients with carotid atherosclerosis after correcting the confounding factors such as diabetes and
hypertension (P<0.05). The predictive equation of ischemic stroke in patients with carotid atherosclerosis was constructed according
to the results of multivariate Logistic regression analysis. The results showed that the incidence of ischemic stroke in patients with
carotid atherosclerosis increased with the increase of score. When the scoring model was 4, the area under the ROC curve was 0.886
(95%CI: 0.791-0.893), and the sensitivity and specificity were 86.94% and 89.12%, respectively. Conclusion LRNC, FCR and the NWI and RI of common carotid artery are risk factors for ischemic stroke in patients with carotid atherosclerosis. The risk earlywarning model for ischemic stroke based on high-resolution MRI has high predictive value.
