摘要目的 比较锥光束乳腺CT(Cone-Beam Breast Computed Tomography,CBBCT)平扫及数字乳腺X线摄影(Digital
mammography,DM)对乳腺病变的诊断效能。方法 回顾性分析2020年11月至2021年1月就诊于郑州大学第一附属医院并
在术前行CBBCT检查与DM检查的乳腺病患者的临床资料,共纳入96例患者,114个病灶,由同一位高年资医生分别评估
CBBCT平扫图像及DM图像,记录腺体分型,影像特征、BI-RADS分类等。结果 CBBCT与DM对乳腺腺体分型具有一致
性,Kappa值为0.642,P<0.01;CBBCT及DM对于病灶的总体检出率(89.5% vs. 70.2%,P<0.01)及良性病变的检出率有显
著差异(86.2% vs. 60.3%,P<0.01),但对恶性病变的检出率无明显差异(92.9% vs. 80.4%,P=0.096)。CBBCT与DM对
乳腺癌诊断的敏感性及特异性无明显差异,但CBBCT平扫AUC值大于DM(0.875 vs. 0.809)。结论 CBBCT平扫有助于提
高病灶的检出率,诊断效能优于DM。
Abstract:Objective To compare the diagnostic efficiency of cone-beam breast computed tomography (CBCCT) and digital
mammography (DM) in breast lesions. Methods The clinical data of breast patients who were admitted to the First Affiliated
Hospital of Zhengzhou University from November 2020 to January 2021 and underwent preoperative CBBCT and DM examination
were retrospectively analyzed. A total of 96 patients with 114 lesions were included. The same senior physician evaluated the
images of CBBCT and DM respectively, and recorded the gland classification, image characteristics, and BI-RADS classification.
Results The classification of breast glands by CBBCT was consistent with that by DM (Kappa value was 0.642, P<0.01). There
were significant differences between CBBCT and DM in the overall detection rate of lesions (89.5% vs. 70.2%, P<0.01) and
benign lesions (86.2% vs. 60.3%, P<0.01) , but there was no significant differences in the detection rate of malignant
lesions (92.9% vs. 80.4%, P=0.096). There was no significant difference in sensitivity and specificity between CBBCT and DM in
the diagnosis of breast cancer, but the AUC value of CBBCT was higher than that of DM (0.875 vs. 0.809). Conclusion CBBCT is
helpful to improve the detection rate of lesions, and the diagnostic efficiency is better than that of digital mammography.
