Abstract:Objective To compare the effects of different concentrations of iodine contrast agent on the quality of enhanced hepatic
vascular CT image under constant total iodine dosein patients with pre-hepatectomy planning. Methods Patients were selected in our
hospital from April 2018 to March 2020, and prospectively and randomly divided into two groups, receiving different concentrations
of contrast agent: 400 mgI/mL (high concentration group) and 300 mgI/mL (low concentration group), respectively, at the same
injection rate and constant total iodine dose. The vascular image quality was assessed quantitatively [HU value, signal to noise ratio
(SNR) and contrast to noise ratio (CNR)] and qualitatively (Likert 4-point method), in portal vein phase, venous phase and
delay phase. Results A total of 128 subjects were enrolled, and 64 cases in each group. The CNR and SNR of the hepatic artery and
abdominal aorta in the arterial phase in the high concentration group were significantly higher than those in the low concentration
group (P<0.05); in the qualitative evaluation, the blood vessel score of the arterial phase image quality in high concentration group
was higher than that of the low concentration group, and the proportion of images score at 3 and 4 points in the high concentration
group was higher than that of the low concentration group (90.62% vs. 51.57%, P<0.001). There were no statistically significant
differences in quantitative and qualitative assessment of blood vessels and liver parenchyma in portal vein, hepatic vein
and delayed phase (P>0.05). Conclusion At the same injection rate and constant total iodine dose, high concentration contrast
agent (400 mgI/mL) can effectively improve the quality of vascular images in arterial phase, which is helpful for preoperative
evaluation of hepatic vascular conditions.
