Preliminary Study on the Improvement of Abdominal CT Angiography Image Quality Based
on Forward Projected Model-Based Iterative Reconstruction Solution

Abstract:Objective To investigate the application of forward projected model-based iterative reconstruction solution (FIRST) in
improving image quality of abdominal CT angiography. Methods A total of 30 patients who underwent abdominal CT angiography
(CTA) in Beijing Hospital were retrospectively analyzed. The images were reconstructed by different algorithms, including filtered
back projection (FBP), adaptive iterative dose reduction 3D (AIDR 3D) and FIRST, and time maximum density projection was used
for postprocessing all the three group of images. The CT values, standard deviation (SD), signal to noise ratio (SNR) and contrast
to noise ratio (CNR) of the major abdominal vessels and muscles were compared using one-way ANOVA, which the main vessels
included the abdominal aorta (AA) , celiac trunk (CT), superior mesenteric artery (SMA), renal artery (RA), and right hepatic artery
(RHA) at the level of the 12th thoracic vertebra. Two radiologic technologists (with 7 and 10 years of experience, respectively)
double-blind evaluated the imaging quality according to a 5-point criterion, and interobserver agreement was analyzed by Kappa
analysis. The Kruskal-Wallis test was used to analyze the differences between the three groups of images. Results SNR and CNR
values of the CT, SMA, RA, and background (muscle) were significantly higher in the FIRST group compared with the FBP and
AIDR 3D groups (P<0.001). There was no significant difference among the three groups in CT values of the CT, SMA, RA, RHA,
and background (muscle) (P>0.05). For subjective scores, both the FIRST group and the AIDR 3D group met the diagnostic needs,
and the subjective scores were better than those of the FBP group (P<0.05). Moreover, the subjective scores of the FIRST group were
significantly better than those of the AIDR 3D group (P<0.05). Conclusion The FIRST reconstruction algorithm can significantly
improve the image quality when compared with FBP and AIDR 3D.
