Dosimetric Comparison of Volumetic Modulation Arc Therapy for
Esophageal Cancer Based on Auto-Planning

Abstract:Objective To explore the advantages of Auto-Planning in volumetic modulation arc therapy (VMAT) for esophageal cancer.
Methods A total of 13 patients with esophageal cancer who were staged for Ⅱ-Ⅲ and underwent concurrent chemoradiotherapy
were selected to complete Manual-VMAT and Auto-VMAT plans in the Pinnacle V9.10 radiotherapy planning system (TPS). The
dose distribution in the planned target area and the exposed dose to the organs at risk (OAR) in the two groups were compared,
and the planned optimization time and the total number of monitor units were also compared. Results Both Manual-VMAT and
Auto-VMAT plans could meet the requirements of clinical prescriptions. The dose conformality and heterogeneity of the targets
in Auto-VMAT plans were slightly better than Manual-VMAT plans, and the overall mean dose of the targets was slightly lower.
Compared with Manual-VMAT plan, the Dmean, V20, V30 of both lungs in Auto-VMAT plan were decreased by 3.1%, 9.0%, and 19.1%,
respectively. Dmean, V30, V40 and spinal cord Dmax were decreased by 12.25%, 23.0%, 24.0% and 3.6%, respectively, with statistically
significant differences (P<0.05). The average optimization time of Auto-VMAT and Manual-VMAT plan was 50 min and 85 min,
respectively. The average number of monitor units was (422.3±60.7) and (381.8±66.7) MU, respectively, and the MUs of Auto-VMAT
plan increased by 9.6% in compared with Manual-VMAT plan (P<0.05). Conclusion Auto-Planning can reduce the exposure dose to OARs
on the premise of ensuring dose coverage of the targets, effectively shorten the time of plan design and improve the efficiency of plan design.
