Comparative Study of Mdixon, DWI and DCE in Preoperative Evaluation of Tongue

Abstract:Objective To explore the application value of spin multi-echo Dixon (mDixon) water/lipid separation imaging combined with anti-motion artifact sequences (MultiVane) in preoperative evaluation of tongue squamous cell carcinoma (SCC), and conduct a comparative study with diffusion weighted imaging (DWI) and dynamic contrast enhancement (DCE). Methods A total of 41 cases of tongue squamous cell carcinoma con?rmed by surgery and pathology in our hospital from January to May 2020 were included as the research objects. All patients were examined by mDixon, DWI and DCE before operation. The signal intensity and background noise of region of interest (ROI) in T2W mDixon and DWI images were measured. Signal noise ratio (SNR) and contrast to noise ratio (CNR) were calculated, and the subjective image quality of T2W mDixon, DWI and DCE was compared and analyzed for tumor maximum diameter, tumor thickness and lymph node metastasis. Results The subjective image quality score of the lesion on the T2W-mDixon sequence was highigest, with showing the best anatomical structure. The SNR and CNR of the T2W-mDixon water maps were higher than their in-phase maps, and the SNR and CNR of the DWI were higher than the T2W-mDixon in-phase maps and water maps. DWI showed the highest proportion of uneven hyperintensity (33/41, 80.5%). It was found by paired comparison that DCE and DWI were closer to the pathological results than the T2W-mDixon water maps in evaluating the maximum diameter and the tumor thickness of the lesion. The threshold of ADC value was 1.20×10-3 mm2/s to identify lymph node metastasis, the sensitivity, positive predictive value, and Youden index were higher than T2W-mDixon, DWI (ADC value threshold 1.15×10-3 mm2/s) and DCE, but the difference in accuracy of four groups were not signi?cant (P>0.999). Conclusion The mDixon technology combined with anti-artifact MultiVane sequence is bene?cial for obtaining stable images in tongue examination. T2W-mDixon, DWI and DCE have their own advantages, the combination of them is helpful to improve the accuracy of preoperative assessment of tongue SCC.
