摘要 目的 评价双源CT血管成像(DSCTA)对成人冠状动脉瘘(CAF)的诊断价值。方法 选取
本院2011年11月~2015年7月间收治的16648例行冠状动脉DSCTA的受检者作为研究对象,通过容
积再现(VR)、多平面重组(MPR)、最大密度投影(MIP)、曲面重组(CPR)等方法对其图
像进行后处理,分析CAF的起源、走行及引流部位。结果 16648例患者中,共诊断CAF44例,其
中右心系统瘘41例,左心系统瘘3例;单瘘口43例,多瘘口1例;单一血管供血32例,多个血管供
血12例。结论 冠状动脉双源CT血管成像简单、安全、无创,可作为CAF诊断的首选方法。

Abstract :Objective To study the effectiveness of application of dual-source computed tomography
angiography (DSCTA) in the diagnosis of coronary artery fistula (CAF) in adults. Methods DSCTA
images of 16648 patients who had been treated in the hospital from November 2011 to July 2015 were
analyzed retrospectively. The DSCTA source imaging data were post-processing reconstructed by volume
rendering (VR), multiple planar reformation (MPR), maximum intensity projection (MIP) and curved
planar reformation (CPR). The origin, course and the site of CAF were analyzed. Results Among the
16648 patients, 44 patients were diagnosed with CAFs by DSCTA, including 41 patients with right
cardiac chamber fistulas and 3 with left cardiac chamber fistulas. Among the 44 patients, 32 patients
had one traceable fistula and the remaining 12 patients were shown to have two or three fistula vessels.
Conclusion Coronary CTA demonstrates itself as a useful, noninvasive and easy-to-operate imaging
modality and should be taken as a first choice for the detection of CAF.
