Abstract:Objective To explore the influence of regions of interest (ROI) selection on the repeatability of energy spectrum CT
parameters of gastric cancer. Methods A total of Forty-five patients with advanced gastric cancer who were treated in our hospital
from May 2018 to October 2020 and planned to receive radical surgical treatment were prospectively selected for this study. All
patients underwent dual-phase energy spectrum CT examination in the abdominal arteriovenous phase one week before surgery. The
circular ROI of near mucosal enhancement area (ROI-10 mm2), the circular ROI of near central enhancement area (ROI-30 mm2)
and the ROI of maximal layer of whole tumor (ROI-outline) were used to delineate and select ROI respectively. The CT value (CT-AP, CTVP),
iodine concentration value (IC-AP, IC-VP), standardized iodine concentration value (NIC-AP, NIC-VP) and the slope of energy
spectrum curve (slope-AP, slope-VP) of gastric cancer in the arterial and venous phase were measured by two observers respectively.
Take the average value of measured parameters by the two observers for comparison. Intra-group correlation coefficient (ICC) and
Bland-Altman method were used to evaluate the consistency and repeatability of measurement results respectively. Results The
parameters of arteriovenous phase and venous phase measured by energy spectrum CT with three different ROI methods were in
good agreement (ICC>0.75). The 95% CI interval of the difference of energy spectrum CT parameters measured by ROI-Outline
method was smaller, and the difference distribution interval of NIC-AP and NIC-VP was more concentrated than that measured by
the other two methods. The maximum-scale total tumor method had better consistency and reproducibility in repeated measurements
across different observers. There were significant differences in the energy spectrum CT parameters measured by three different ROI
methods (P<0.05). The parameters measured by ROI-10 mm2 method were higher than those measured by ROI-30 mm2 method, and
the parameters measured by ROI-30 mm2 method were higher than those measured by ROI-Outline method. The difference between
the groups was statistically significant (P<0.05). Conclusion Different selection of ROI will affect the parameters value of energy
spectrum CT for gastric cancer. The energy spectrum CT parameters measured by the maximum-layer whole-tumor method have the
best repeatability and can be better used for the evaluation of clinical effect for gastric cancer.
