Value of the Axillary Lymph Nodes of Breast Cancer Ultrasound Assessment

Abstract:Objective To evaluate the value of ultrasonography in diagnosis of axillary lymph node (ALN) metastasis in breast cancer.
Methods A retrospective analysis of 265 cases of axillary lymph nodes who were diagnosed as breast cancer admitted in the first
affiliated hospital of nanjing medical university from January 2016 to April 2017 were included in this study. The degree and the
state of the lymph nodes of cortical thickening and the hilus structure in bilateral were analyzed. Results Nomal axillary lymph
nodes were found in 132 cases, 20 with pathological metastasis. 133 with unnormal axillary lymph nodes, among them, 77 were
found with pathological metastasis. The sensitivity, specificity, positive predictive value, negative predictive value were 79.38%,
66.67%, 57.89% and 84.85% respectively. These patients were then grouped according to the cortical thickening of 3.0 mm. As a
result, patients with cortical thickness ≥3.0 mm had higher lymph node metastasis rate. In particular, the metastasis rate was 100%
in lymph node with disappearance hilus (P<0.05). But in the same group of cortical thickness uniform and non-uniform, there is no
obvious difference for lymph node metastasis (P>0.05). Ultrasonic normal group (false negative) had lower metastatic nodal burden
than that of cortical thickness ≥3.0 mm group (P<0.05). Conclusion Ultrasound can effectively evaluate axillary lymph node status
in breast cancer. The metastasis rate is higher in cortical thickness ≥3.0 mm than that of cortical thickness <3.0 mm.
