Advantage of Dual Energy Subtraction Technique in Elderly Pulmonary Examination

Abstract:Objective To investigate the clinical value of X-ray digital radiography (DR) dual energy subtraction (DES) for elder
people during lung examination. Methods We selected 435 cases over the age of 55 who participated in physical examination in 2016
in our hospital. All these patients underwent DES photography. The standard DR (SDR), soft tissue subtraction DR images, and bone
subtraction DR images were obtained and were analyzed by a chief physician and 2 deputy chief physicians of imaging diagnosis.
Results Compared with the SDR image, the DES image significantly increased the detection rate of pulmonary nodules (c2=7.212,
P=0.007). If the pulmonary nodules sited in the overlap of lung tissue and rib, the image will clearer after subtraction. However, if
the pulmonary nodules sited between the ribs, the image will less clear after subtraction. The detection rate of pulmonary nodules was
much higher in DES image than that of SDR image in different smoking ages, but the differences were not significant. Conclusion
DES technique improves the detection rate of pulmonary nodules by removing interference of ribs and reducing the effect of
breathing. Therefore, this technique could be used for routinely physical examination for elderly people with smoking history.
