Application of Ultrasound ABVS and VTI in Differential Diagnosis of Breast Space-Occupying Lesions

Abstract:Objective To investigate the value of ultrasonic automated breast volume scanner (ABVS) and virtual touch tissue
imaging(VTI) in the differential diagnosis of breast space-occupying lesions. Methods A total of 169 patients with breast space
occupying lesions were selected as the research subjects, of which 120 cases were benign lesions and 49 cases were malignant lesions
confirmed by postoperative pathology. The two-dimensional ultrasonography, ABVS and VTI were performed before operation. The
positive predictive value (PPV) and negative predictive value (NPV) were calculated by the four-table method. The receiver operating
characteristic (ROC) curve model was used to analyze the area under the curve (AUC) value, sensitivity and specificity of breast
space-occupying lesions of two-dimensional ultrasound, ABVS, VTI, and ABVS combined with VTI. Results The PPV and NPV
were 54.23% and 84.55%, respectively after two-dimensional ultrasonography in 169 patients with breast space occupying lesions.
Compared with the gold standard, the consistency was general (Kappa=0.404, P<0.001). After ABVS examination, the positive
predictive value was 82.00%, the negative predictive value was 93.28%, and the consistency was general compared with the gold
standard (Kappa=0.757, P<0.001). After VTI examination, the positive predictive value was 70.59%, and the negative predictive
value was 88.98%. Compared with the gold standard, the consistency was general (Kappa=0.607, P<0.001). After ABVS and VTI
examination, the positive predictive value was 95.92%, and the negative predictive value was 98.33%. Compared with the gold
standard, the consistency was better (Kappa=0.943, P<0.001). Among them, the detection rate of two-dimensional ultrasound was
lower than that of ultrasound, ABVS, VTI, and the combination of the two methods (P<0.001); there was no significant difference
between the detection rate of ultrasound ABVS and VTI (P=0.078). The detection rate of ABVS was lower than that of combination
of the two methods (P<0.001). The detection rate of VTI was lower than that of combination (P<0.001). Among the 41 patients with
malignant lesions detected by ABVS, there were 20 cases with “crater” sign changes, 17 cases with “crab group” changes, and 4
malignant cases without the above changes. Among the 36 patients with malignant lesions detected by VTI, there were 14 patients
with elasticity score of 4, and 22 patients with elasticity score of 5. The ROC curve analysis showed that the AUC values of twodimensional
ultrasound, ABVS, VTI, and the combination of the two methods in identification of breast space-occupying lesions were 0.714, 0.881, 0.805, 0.971 respectively (P<0.001). Conclusion Ultrasound ABVS and VTI both have certain application value in the
differential diagnosis of breast space-occupying lesions, but the combination of them is beneficial to improve the detection rate.
