Evaluation of Extravascular Lung Water Before and After Continuous Blood Purification by
B-Line of Pulmonary Ultrasound in Patients with MODS

Abstract:Objective To determine whether the number of B-line of lung ultrasonography can accurately evaluate the extravascular
lung water (EVLW) in patients with multiple organ dysfunction syndrome before and after continuous blood purification (CPB).
Methods A total of 60 patients with MODS treated with CBP were evaluated for EVLW 24 h before and after CBP treatment
by using the number of B-line of pulmonary ultrasound, and compared with the EVLW of percutaneous pulse-induced contour
cardiac output (PiCCO). The correlation was analyzed between the change of the number of B-line before and after CBP treatment
and the negative liquid balance at 24 h. Results The average number of B-line before CPB treatment was (25.9±7.3), which
was significantly less than that after treatment (16.2±6.2) ( P<0.05). The EVLW before CPB treatment was (18.2±6.4) mL/kg,
which was significantly lower than the EVLW after CPB treatment (12.8±5.3) mL/kg (P<0.05). According to Pearson correlation
coefficient analysis, the number of B-line before and after CPB treatment was positively correlated with EVLW (r=0.78, 0.72,
P<0.05). The number of B-line before and after CPB treatment was negatively correlated with oxygenation index (r=-0.742,
-0.706, P<0.05). The number of B-line reduction before and after CBP treatment was positively correlated with the amount of net
dehydration in 24 h (r=0.65, P<0.05). Conclusion There was a good correlation between the number of lung ultrasound B-line
changes and the EVLW obtained by PiCCO. The lung ultrasound B-line can be used to evaluate the changes in EVLW before and
after CBP treatment.
