Dosimetry Comparison Between Automated and Manual Volumetric Modulated Arc Therapy

Abstract:Objective To compare the dosimetry difference between automated volumetric modulated arc therapy planning (Auto-VMAT)
and manual volumetric modulated arc therapy planning (Manual-VMAT) of Pinnacle3 9.10 treatment planning system using the standard dose
verification module and the dose target as the objective evaluation criteria, and to guide the use of auto-plan of Pinnacle planning system.
Methods We designed Auto-VMAT and Manual-VMAT with Pinnacle3 9.10 treatment planning system for each standard phantom used
in AAPM 119 report. The differences of dose volume histogram in organs at risk and target volume, target’s conformal index (CI), target’s
homogeneity index (HI), times of artificial debugging, planning time as well as monitor unit (MU) were compared between Auto-VMAT and
Manual-VMAT. Results Auto-VMAT could reduce the hot spot of target, artificial debugging times and planning time. The CI and HI of Auto-VMAT
were better than Manual-VMAT, except for the central target in multi-target phantom. There was no significant difference of the dose of OAR
between Auto-VMAT and Manual-VMAT. The MU of Auto-VMAT was less than Manual-VMAT in head-neck and C shape phantom, while the
result was opposite in multi-target and prostate phantom. Conclusion Auto-VMAT can get better radiotherapy plan than Manual-VMAT. On the
other side, it can reduce the optimized time of plan and avoidt he human error in designing the plan and diference between different physicists.
