Abstract:Objective To investigate the clinical value of temporal bone thin layer scanning of 256 slice spiral CT in assessing
bone destruction before tympanoplasty. Methods From August 2017 to August 2018, 50 cases of otitis media with cholesteatoma
treated by mastoid lesions resection combined with tympanic room plasty in our hospital were selected as subjects of this
study. All the patients were performed with HRCT scan of the temporal bone axis before surgery. MPR and CPR were used to
reconstruct and compare the results of the preoperative examination with the postoperative pathological diagnosis to evaluate
the diagnostic accuracy of the 256-row helical CT thin slice scan of the temporal bone. Results According to the temporal bone
thin layer scanning, the diagnostic coincidence rate of the destruction of middle ear bone was 100.0%, 95.3% of shield plate, 92.9%
of the posterior wall of external auditory canal, 100.0% of semicircular canal, 92.9% of the roof of tympanic room and 84.6% of the
opposite neural tube. According to the auditory chain of patients, the diagnostic coincidence rate of anvil short foot destruction was
93.8%, anvil long foot 97.6%, mallet anvil joint 100.0%, mallet anvil joint 94.1%, anvil body 90.0%, mallet shank 100.0%, stapes
bone 100.0%, anvil joint 95.6%. Conclusion The reconstruction of coronal images using HRCT and MPR of the temporal
bone axis with 256 row helical CT can accurately show the destruction of middle ear bone, the structure and connection
of auditory chain, the degree and range of lesions. The scanning technology has high clinical practicability.
