摘要

目的 探讨分层应变技术评估绝经期妇女左心室收缩功能的改变。方法 连续性选择绝经期妇女41例以及年龄匹配 的非绝经期妇女46例纳入研究。测量所有受试者左心室18节段整体平均纵向心内膜应变(Globle Longitudinal Strains of the Endocardium,GLS-endo)、中层心肌应变(Globle Longitudinal Strains of the Myocardium,GlS-myo)以及心外膜应 变(Global Longitudinal Strain in the Epicardial Layer,GLS-epi)。结果 两组左心室射血分数、左室舒张末期容积、左室收 缩末期容积以及室间隔和左心室后壁厚度无统计学差异(P>0.05);两组分层应变均表现为从心内膜向心外膜逐渐递 减趋势(P<0.001)。与绝经前组比较,绝经后组GlS-endo显著减低(P<0.001),两组GlS-myo和GlS-epi无统计学差异 (P>0.05)。结论 左心室分层应变的评估,为绝经期妇女心脏功能特定层的损害的认识提供了新的视角。

Abstract

Objective The aim of this study is to assess the functional change of the left ventricular longitudinal by using layerspecific strain in post-menopause women. Methods A total of 41 post-menopause women were included, and the control group included 46 age-matched pre-menopause women. The global of longitudinal strains of the endocardium (GLS-endo), myocardium (GLS-myo), and epicardium (GLS-epi) were obtained by averaging peak systolic strain of the left ventricle, with 18 segments in each layer. Results The left ventricular ejection fraction, left ventricular endsystolic volume, and left ventricular end-diastolic volume were similar between post-menopause women and pre-menopause women (P>0.05). Layer-specific strains showed a decreasing gradient from the endocardium to the epicardium in both groups (P<0.001) and were significantly different between post-menopause women and pre-menopause women in endocardium (P<0.05). Both GLS-myo and GLS-epi were no significantly different between post-menopause women and pre-menopause women (P>0.05). Conclusion Layer-specific evaluation of the left ventricle provides an understanding of the layer-specific properties of the left ventricle wall and the process of left ventricle impairment, and it may be helpful in managing post-menopause women.
