Abstract:Objective To investigate the value of contrast-enhanced ultrasonography in diagnosis of small hepatocellular carcinoma
(HCC). Methods A retrospective analysis including 35 cases with lesions less than 1.5 cm in the department of ultrasound the fifth
hospital of Shijiazhuang was performed, and the characteristics of two-dimensional gray scale ultrasound and contrast-enhanced
ultrasound were analyzed. Results The size of lesions showed that 9 lesions were less than 1 cm, 5 lesions were 1.1 cm, 5 lesions
were 1.2 cm, 6 lesions were 1.3 cm and 10 lesions were 1.4 cm. The number of lesions with high-echo, iso-ehco and low-ehco were 8,
1 and 26, respectively. Among the patients with low-ehco, 6 were accompanied with faint. Contrast-enhanced ultrasonography (CEUS)
showed that there were 18 lesions presented fast forward and rewind, 6 lesions presented fast forward and no rewind, 1 lesions
presented equal enhancement to exit, 1 lesion showed low to enhancement to low enhancement, 4 lesions showed enhancement,
3 lesions presented low enhancement to enhancement and 2 lesions showed low enhancement. The results of aspiration biopsy
showed that there were 3 hemangiomas, 3 regenerative nodules, 5 atypical hyperplasia nodules, 22 HCC, 1 adenocarcinoma, and
1 cholangiocarcinoma. The sensitivity, specificity and accuracy in the diagnosis of small hepatocellular carcinoma were 50.4%,
66.7% and 45.7% for of conventional ultrasonography, and 89.6%, 66.7%, and 62.8% for contrast-enhanced ultrasonography
respectively. The sensitivity and accuracy of contrast-enhanced ultrasonography in diagnosis of small hepatocellular carcinoma were
higher than that of conventional ultrasound. Conclusion Compared with conventional ultrasound, CEUS can better display hepatic
microvascular perfusion characteristics of small hepatocellular carcinoma, so it is effective in identification of benign and malignant
lesions. Therefore this methods is of high application value and development prospects in improving the accuracy of diagnosis of
small hepatocellular carcinoma and small hepatocellular carcinoma.
