Clinical Study of Thermoplastic Body Film Deformation and Its Impact on Radiotherapeutic

Abstract:Objective To evaluate the relation between cooling time and contraction of thermoplastic body film using for fixation of
female pelvic tumors, to observe the change rule of body film deformation on the displacement of radiotherapy positioning marker, so
as to clarify the potential stability differences of thermoplastic body film fixation sites at two different observed time points.
Methods Thirty patients with advanced cervical cancer who were to receive intensity-modulated radiotherapy in the radiotherapy
center of our hospital from December 2021 to March 2022 were selected and fixed by vacuum pad and thermoplastic body film. The
positioning center points of the Y-axis were placed 3 cm above the umbilicus (point A), horizontally at the umbilicus (point B), and
3 cm below the umbilicus (point C). The X-axis was placed on the body midline and the Z-axis was placed on the axillary midline.
A three-dimensional laser system was used to mark the cross laser on the body membrane. Based on the “cross” markers as baseline
drawn after 20 minutes cooling, the three-dimensional space displacement of the three positioning points of the thermoplastic body
film at the 4th and 24th hours was observed and recorded. Results Among the three observation locations, 23.3%, 20.0% and 23.3%
of the cases were offset >5 mm on the X-axis at the 4th hour, and 0 cases on the Y-axis and Z-axis. In addition, 13.3% of the three
observation sites had a shift >5 mm on the X-axis at the 24th hour, 1 case on the Y-axis and 0 case on the Z-axis. The film shrinkage of
thermoplastic body was significantly different among the three observation points (point A, point B, point C) at different observation
time points on the X-axis (P<0.05). Conclusion In the fixation of female pelvic tumor with thermoplastic film, the simulation
position should be performed at least 4 hours after the production of body film, and it will be a benefit if it is performed 24 hours
after the production.
