Correlation of Venous Thrombosis and Thrombolytic Effects on Hardness of
Real-Time Shear Wave Elastography Technology

Abstract:Objective To quantitatively evaluate the hardness of deep venous thrombosis of lower extremity by using the shear wave
elastography (SWE) technique, and to identify the relationship between the thrombolytic effect and thrombus hardness by different
thrombolytic regimens (such as catheter directed thrombolysis, systemic thrombolysis) and the young’s modulus pathways. Methods
A total of 97 cases with symptoms of acute central complete venous thrombosis and definite venous thrombosis diagnosed by regular
ultrasound, were included in this study. Patients were divided into cathetering thrombolysis group and systematic thrombolysis
group. After 7-10 days of anticoagulation or thrombolytic therapy, patients in the two groups were further divided into cure group,
effective group and ineffective group according to the different treatment result. The total cure rate and Young’s modulus value
were compared between the two groups. Besides, the patient’s work characteristic curve was established to determine the effective
critical value of the young’s modulus. Results The Young’s modulus in cure group, significant effective group and ineffective group
in the two groups were significantly different (P<0.01), while that between the cure group and effective group of Young’s modulus
was not significant (P>0.05). The sensitivity and specificity of Young’s modulus at 8.50 kPa was 92% and 93.7% respectively. The
difference of the young’s modulus between the cathetering thrombolysis group and systematic thrombolysis group was statistically
significant (P<0.01). Conclusion The young’s modulus of the real time SWE can quantitatively evaluate the thrombus hardness,
and the hardness of thrombus is related to the thrombolytic effect. Therefore, the young modulus can predict the thrombolysis
effect. According to the young’s modulus of dissolved thrombus, the effect of thrombolytic therapy is better than that of systemic
thrombolysis.
