Abstract:Objective To evaluate the value of second harmonic contrast technology in diagnosis of uterine fallopian tube patency
and function. Methods In 500 cases of infertility patients (882 oviduct) by transvaginal four-dimensioalhysterosalpingocontrast sonography (TVS 4D-HyCoSy) inspection, after the vagina second harmonic uterus fallopian tube inspection, record
the tubal patency, direction, the umbrella end development situation, choose the contrast-enhanced ultrasound tip side or both
sides impassability, 80 cases (152 oviduct) accepted laparoscopic methylene blue fluid examination, contrast analysis. Then two
kinds of ultrasonic salpingography were used to compare the fallopian tube walking and morphological assessment. Results The
coincidence rate of second harmonic combination TVS 4D-HyCoSy patency in 80 patients (152) was 93%, and the coincidence
rate of oviduct obstruction was 96%. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of
TVS 4D-HyCoSy in evaluating fallopian tube patency were 95%, 96%, 95%, 97% and 93%, respectively. The coincidence degree of
TVS 4D-HyCoSy was highly consistent with that of laparoscopic methylene blue drainage, with a Kappa value of 0.85. Compared
with laparoscopy, the coincidence rate of tube umbrella end patency was 97% and the coincidence rate of obstruction was 85%. The
coincidence rate of tubal coil was 78%, the coincidence rate of thickness unevenness was 64%, the coincidence rate of refolding
was 73%, the coincidence rate of umbrella end inclusion effusion was 80%, and the coincidence rate of umbrella end elongation was
84%. Compared with laparoscopy, TVS 4D-HyCoSy was used to evaluate the coincidence rate of tubal coil (69%), uneven thickness
(48%), refolding (44%), inclusion effusion (20%), and elongation (68%). 420 patients (775) were diagnosed with bilateral patency in
203 cases (406 cases), and 121 cases were naturally pregnant. There were 104 cases (208 cases) of bilateral impassability, 4 of which
were conceived spontaneously and 20 of which were conceived in vitro. There were 113 cases (161 cases) of unilateral impassability,
47 cases of natural conception and 11 cases of in vitro fertilization. Conclusion The second harmonic is of high diagnostic value
in the patency of uterine fallopian tube, especially for the accurate evaluation of the abnormal shape, thickness and umbrella end
adhesion of the fallopian tube, which can help clinical evaluation of the function of the fallopian tube more comprehensively.
