Study on the Value of 3.0 T Magnetic Resonance IVIM Imaging in Differentiating Primary
Benign and Malignant Pulmonary Lesions

Abstract:Objective To explore the application value of 3.0 T magnetic resonance intravoxel incoherent motion (IVIM) imaging in
the identification of primary benign and malignant lung lesions. Methods The clinical and imaging data of 40 patients with primary
lung space-occupying lesions were retrospectively analyzed. According to the pathological results, they were divided into benign
group (12 cases) and malignant group (28 cases). All patients underwent IVIM-DWI examination before operation. The b values were
0, 100, 200, 400, 600, 800, 1000, and 1500 s/mm2. The sADC value, D value, D* value and ? value of all lesions were measured.
The difference of parameters between the two groups was compared, and the receiver operating characteristic curve was drawn to
evaluate the diagnostic efficiency. Results The sADC values of benign group and malignant group were (1.65±0.55)×10-3 mm2/s
and (1.29±0.41)×10-3 mm2/s丨 D values were 1.17±0.47×10-3 mm2/s and (0.86±0.32)×10-3 mm2/s, respectively. D* values were
(26.34±30.10) ×10-3 mm2/s, (50.67±47.21)×10-3 mm2/s丨 ? values were (42.50±24.78)% and (33.96±15.64)%, respectively. The
difference in sADC value, D value, and D* value between the two groups was statistically significant (P<0.05), while the difference
in ? value was not statistically significant (P>0.05). The AUC of the sADC value, D value, and D* value were 0.79, 0.81, and 0.70,
the sensitivity was 89.3%, 91.8%, and 77.6%, and the specificity was 58.1%, 60.3%, and 67.2%, respectively. Conclusion 3.0 T
magnetic resonance IVIM imaging has certain guiding significance in the identification of primary lung benign and malignant spaceoccupying
lesions, which can reflect both tissue diffusion information and perfusion information.
