Abstract:Objective To explore the clinical value of acoustic radiation force impulse (ARFI) combined with shear wave
elastography (SWE) in the diagnosis of BI-RADS4 breast lesions. Methods A total of 105 cases of BI-RADS type 4 breast
lesions diagnosed by preoperative ultrasonography were selected from the patients treated in our hospital from August 2016 to
May 2018. ARFI (including VTI and VTQ) and SWE examinations were performed. Data were analyzed statistically and ROC
curves were drawn. Results In ARFI, the VTI score (0.97±0.07) of benign lesion was lower (P<0.05) than that of malignant lesion
(4.36±0.51). In VTQ, the average shear wave velocity of benign lesions (2.18±0.47) m/s was lower (P<0.05) than that of malignant
lesions (7.32±3.05) m/s. In SWE, the maximum elastic modulus value and mean elastic value of malignant lesions were significantly
higher than that of breast gland tissue and benign lesions (P<0.05). The specificity (82.5%) and accuracy (77.1%) of combined diagnosis of
ARFI and SWE were higher than those of single detection. The area under ROC curve was 0.875 (95% CI: 0.835-0.915) in the combined
diagnosis, which was significantly higher than that in the single detection (P<0.05). Conclusion ARFI combined with SWE can
improve the diagnostic value of BI-RADS4 breast lesions.
