Diagnostic Value of Intravoxel Incoherent Motion of Diffusion Weighted Imaging in
Differentiating Benign from Malignant Breast Lesions and Pathological Grading Studies

Abstract:Objective To investigate the diagnostic value of parameters derived from diffusion-weighted imaging intravoxel
incoherent motion (IVIM) for benign and malignant breast lesions and the pathological grading studies of breast lesions. Methods
A total of 80 cases of breast disease with breast MRI examination in our hospital were collected and all of them were confirmed
by surgical pathology and clinical datas. All of them were examined with multiple-b-value diffusion-weighted imaging (DWI), the
value of ADC, ADCfast, ADCslow and f were calculated. The above parameters between breast benign and malignant lesions were
compared, the effect in different pathological grading of breast lesions were also compared. The diagnostic efficacy of ADC value,
ADCslow and f values in benign malignancy were compared by drawing the characteristic curve of the subject (ROC curve). Results
The difference of ADC values, ADCslow and f values of benign and malignant breast lesions were statistically significant (P<0.05).
The difference of ADCfast between benign and malignant breast lesions were not statistically significant (P>0.05). Area under the
curve obtained from diagnostic experiment of ADCslow, ADC and f values was in statistical difference with their AUC article which
were 0.917, 0.872 and 0.744, respectively. ADCslow had the best diagnostic effect. The optimal cut-off value of the three diagnosing
breast gland’s benign and threaded lesions was respectively 0.92×10-3 mm2/s, 1.13×10-3 mm2/s and 5.78%. The sensitivities of
benign and malignant breast gland examined by this threshold value were 91.2%, 86.4% and 70.6%, respectively. The difference
of ADC values, ADCslow between breast dysplastic malignancies and poorly differentiated malignancies were not statistically
significant (P>0.05). The difference of ADCfast and f values between breast dysplastic malignancies and poorly differentiated
malignancies were statistically significant (P<0.05). Conclusion Intravoxel incoherent motion of diffusion weighted imaging are of
great value in differentiating benign and malignan breast lesions and pathological grading studies.
