Clinical Value of Diuretic Ultrasound and Enhanced CT in the Diagnosis of Parapelvic Cyst

Abstract:Objective To investigate the clinical value of diuretic ultrasound and enhanced CT in the diagnosis of parapelvic cyst.
Methods A total of 37 cases of parapelvic cysts with diameter>4.0 cm were selected by conventional ultrasound, which underwent
diuretic ultrasound, enhanced CT examination and ultrasound-guided flexible ureteroscopy. Taking the results of flexible ureteroscopy
as the gold standard, the results of diuretic ultrasound and enhanced CT were compared by Kappa consistency analysis. The receiver
operator characteristic (ROC) curve was performed to evaluate the feasibility of diuretic ultrasound combined with enhanced CT in
the diagnosis of parapelvic cyst. Results Among the 37 cases in this study, diuretic ultrasound diagnosed 30 cases and excluded 7
cases, enhanced CT diagnosed 31 cases and excluded 6 cases, flexible ureteroscopy diagnosed 32 cases and excluded 5 cases. The
Kappa coefficients were 0.369 between diuretic ultrasound and flexible ureteroscopy, and 0.467 between enhanced CT and flexible
ureteroscopy, the consistency was existed in both cases. The sensitivity and specificity were 87.5% and 80.0%, respectively, and
the area under the ROC curve was 0.863 when came to diagnosis of parapelvic cyst by diuretic ultrasound combined with enhanced
CT. Conclusion The accuracy of diuretic ultrasound and enhanced CT in the diagnosis of parapelvic cyst is limited, but combined
examination of the two methods can effectively improve the diagnostic efficiency of parapelvic cyst, which has a certain value in the
clinical diagnosis of parapelvic cyst.
