Efficacy Analysis of Breast Imaging Reporting and Data System-Ultrasound
Classification in the Diagnosis of Non-Lactation Mastitis

Abstract:Objective To analyze and summarize the reasons for the classification deviation between different pathological types
of non-lactation mastitis (NLM) and breast imaging reporting and data system-ultrasound (BI-RADS-US), in order to deepen the
sonographer’s understanding of NLM and improve the accuracy of diagnosis. Methods A retrospective analysis of 84 patients
with NLM confirmed by puncture or surgery in our hospital was performed. The BI-RADS classification, pathological type and
ultrasound classification of the lesions were recorded, and the accuracy of BI-RADS classification in each group was calculated and
compared. Results The pathological types of NLM in 84 patients were acute and chronic inflammation, suppurative inflammation,
granulomatous lobulitis and chronic inflammation. According to the ultrasonographic features of NLM, NLM was divided into
ductal dilatation type, nodular type and diffuse type. The accuracy of BI-RADS classification of NLM with different pathological
types was 65.0% for acute and chronic inflammation, 100% for purulent inflammation, 61.1% for chronic inflammation, and 85.7%
for granulomatous lobulitis; there was no significant difference between the different groups (P>0.05). The accuracy of BI-RADS
classification of NLM with different ultrasound types was 100% for ductal dilatation type, 44.8% for nodular type, and 90.9% for
diffuse type, and the difference between different groups was statistically significant (P<0.001). Conclusion The difference of
NLM pathological type and ultrasound classification is the main reason for the classification deviation of BI-RADS. The diffuse and
flowing punctate echoes in the lesions may be the characteristic ultrasound manifestations of NLM.
