Abstract:Objective To study the application value of transcranial Doppler ultrasound (TCD) combined with somatosensory evoked
potential (SEP) in the clinical diagnosis of brain death and the method of image optimization. Methods A total of 120 cases of brain
death in ICU ward of our hospital from April 2015 to April 2017 were selected. All the patients were randomly divided into three
groups: TCD group, SEP group and combined group, each group with 40 cases. Transcranial Doppler ultrasonography was performed
separately on patients of TCD group. Somatosensory evoked potential (SEP) test was performed on patients in the SEP group. Patients in
the combined group were tested with TCD combined with SEP. Results The diagnostic accuracy of brain death in TCD group was
57.50%, that in SEP group was 65.00%, and that in combination group was 82.50%. The diagnostic accuracy of combined group was
significantly higher than that of single brain death group and SEP group (P<0.05). There was no significant difference in misdiagnosis rate
among the three groups (P>0.05). There were no statistically significant differences in sensitivity, specificity and accuracy between
the TCD group and SEP group (P>0.05). The sensitivity, specificity and accuracy of the combined test were significantly higher than
those of the separate tests (P<0.05). Conclusion In patients with brain death, TCD combined with SEP test can make a better
and more accurate diagnosis of brain death, providing theoretical basis for clinicians to comprehensively evaluate the condition
and prognosis of patients with brain death, which is of great c linical significance.
